Patient Registration
Registration Forms

If you are new to the Alresford area and would like to register with our practice please call into the surgery and collect the relevant forms. Upon registration you will be entitled to see any of our doctors, however, if you have a preference for a particular doctor please let our receptionists know. The surgery is accessible to wheelchair users and has suitable toilet facilities.

The Department of Health has asked us to obtain identification from patients wishing to be seen at the surgery. This has been introduced to combat increasing levels of identity fraud and use of NHS services by people who are not entitled to do so. We are sorry for any inconvenience this may cause.  People over 16 wishing to register will be asked to provide photographic proof of identity (eg passport, drivers licence, ID card, or birth certificate if) and a document confirming their address (eg utility bill, tenancy agreement, mortgage document,)

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
