Better Health Clinics
At the Alresford Surgery
The number of people with long-term health conditions (asthma, high blood pressure, diabetes, heart disease etc.) has increased over recent years. Regular and individualised health care has been proven to improve the health of people living with long-term conditions. Many people have more than one condition and find they are invited to attend multiple clinic appointments through the year. To address this we have designed “Better Health Clinics” .
These new clinics have three main aims:
- Provide regular supervision and treatment for your health conditions.
- Ensure that the care provided is individualised to your needs.
- Reduce duplication of tests and the need for multiple blood test appointments and reviews.
Initially people with one or more conditions such as high blood pressure, heart disease, stroke, diabetes, asthma and COPD will be invited to attend. As the system settles in we hope to continue to develop these clinics to involve more people with long-term conditions.
The clinics will provide an annual ‘MOT’ of your long-term health conditions. Your doctor will have made sure that the new clinic is suitable to look after your heath conditions. If this is the case you will be invited to attend an extended appointment with a health care assistant or practice nurse to review your test results and talk about ways you can improve your health. Any investigations such as blood tests will need to be organised before your clinic appointment so that we can have the results before we see you. Although you will not be seeing your doctor for these appointments you can be reassured that they will be overseeing your results and any changes to your medications. If you are already attending our COPD and diabetes clinics, you will be called in the usual way to clinic but the aim is to cover your long-term conditions in one or two appointments. If your health conditions are well controlled you will be seen once a year. Should there be any problems with your condition your GP or nurse may arrange an extra follow up appointment as necessary to ensure optimum control of your condition.
The new system
We believe that this new system will provide a more thorough and individualised approach to your healthcare. As you can imagine there are several thousand people to review and the logistics of changing the way we work has been somewhat of a challenge. We have initially decided to call people for whom the new clinics are suitable, near their birth month. This may mean you are invited to attend at a time you were not expecting. Please bear with us, as there will be some replication of work in the first year to try and incorporate all your reviews into one appointment. There may be some teething problems with this organisational change and we ask you to be patient should this happen. We endeavour to make the change over as smooth as possible.
